The written report you get at the end of an assessment. You are entitled to your own copy, and you can ask for factual mistakes to be put right.

After an assessment, the service writes up what was done, what was found, and what it recommends. Some services send SEND SEND is England's word for children who need extra help to learn. The rest of the UK uses other words, and other rules. Read the full definition of SEND a full report and some send only a short outcome letter, and schools tend to act on what they can read, so it is worth asking for the fullest version available. You are entitled to a copy of your child's report, and you can ask for factual mistakes, such as names, dates or events, to be corrected. If nothing has arrived, chase it: reports can take weeks, and asking when to expect yours is entirely reasonable.
